Leaving Hospital
When you are leaving hospital you will be given information to help your further recovery and you will be told who to contact in an emergency.
We will give you as much notice as possible about when you will be leaving hospital, but we hope you understand that there may be occasions when the notice may be short.
If you have made arrangements to go elsewhere than home, please give your temporary address to a member of the nursing team caring for you.
Getting Home
If possible, please arrange for someone to collect you from hospital and take you home when you are ready to leave. If, due to your condition you need hospital transport, we are able to arrange this with 24 hours notice.
Moving to another Ward or Hospital
You may move to another ward or hospital to continue your treatment and recovery. If this happens, we will give you as much notice as possible and tell your family/friends/carers that you are being moved.
Contact with your GP
Your medication may be changed while in hospital. You will be given 28 days supply of medication but you will need to visit your GP if any repeat prescriptions are requested. Your GP will be sent a letter about your care in hospital and any changes to your medication.
