To sign up as a member with the hospitals, please click here to download the Membership Form.  On this form, you can also become a member with other NHS organisations and influence other areas of your NHS. Members of hospital staff do not need to fill in this form to become a member of the Trust, but are very welcome to use it to joint partner NHS organisations.

All completed forms should be returned to foundation.trust@hey.nhs.uk  your membership will be confirmed within two weeks. Please call 01482 675165 if you have any queries or to fill out a membership form over the phone.
